Overview

These Terms of Service (“Terms”) govern your use of the website and services provided by Arilon Corp d/b/a MOY Health (“MOY Health,” “we,” “us,” or “our”) at trymoy.com. By accessing or using our website, platform, or services, you agree to these Terms.

Eligibility

• You must be at least 18 years old to use MOY Health services.

• Services are available only to residents of the United States.

• By using our services, you represent and warrant that all information you provide is accurate, complete, and current.

Service availability

MOY Health services are available to residents of all 50 U.S. states. Treatment is provided only by physicians licensed in the state where the patient is located, and medications are dispensed only by pharmacies authorized to ship to that state. Services are not available outside the United States.

Nature of Services

MOY Health is a telehealth platform that facilitates access to licensed physicians and licensed U.S. compounding pharmacies. MOY Health does not itself practice medicine. All clinical decisions, including prescribing, are made solely by licensed physicians. Completion of our assessment does not guarantee eligibility for treatment or the issuance of a prescription.

Compounded Medications

Some prescriptions facilitated through MOY Health may involve compounded medications. Compounded medications are not FDA-approved and have not undergone FDA review for safety, effectiveness, or manufacturing quality. They may be lawfully prescribed when a licensed physician determines they are clinically appropriate.

Subscriptions and Billing

• MOY Health offers subscription-based access to telehealth services and medication fulfillment.

• Subscription fees are charged on a recurring basis as described at the time of purchase.

• By providing payment information, you authorize MOY Health to charge your payment method on the applicable billing cycle.

• Pricing is subject to change with advance notice to existing subscribers.

Cancellations and Refunds

You may cancel your subscription at any time through your patient portal or by contacting support@trymoy.com. Cancellation takes effect at the end of the current billing period. Refund eligibility is governed by our Refund Policy available at trymoy.com/refund-policy.

Patient Responsibilities

By using MOY Health services, you agree to:

• Provide accurate and complete health and personal information at all times

• Notify your physician promptly of any changes to your health, medications, or circumstances

• Follow the treatment plan and instructions provided by your licensed physician

• Use MOY Health services only for lawful purposes

• Not share your account credentials or allow others to access your account

Prohibited Conduct

You agree not to misuse or interfere with the operation of our platform, attempt to gain unauthorized access to any part of our systems, provide false or misleading information, or use our services for any unlawful purpose.

SMS and Communications

By providing your phone number you consent to receive text messages from MOY Health related to your account, treatment, and services. You may opt out at any time by replying STOP to any message or contacting us at support@trymoy.com.

Disclaimer of Warranties

MOY Health provides its platform and services on an as-is and as-available basis. We make no warranties, express or implied, regarding the accuracy, completeness, or fitness for a particular purpose of our services.

Limitation of Liability

To the maximum extent permitted by applicable law, Arilon Corp (d/b/a MOY Health) shall not be liable for any indirect, incidental, consequential, or punitive damages arising from your use of or inability to use our services.

Governing Law

These Terms are governed by the laws of the State of Wyoming. Any disputes arising under these Terms shall be resolved in the courts of Sheridan County, Wyoming.
